Be honest with yourself. There is no right or wrong, or good or bad about this questionnaire. It is designed to help you think about where you are right now, and where you would like to be.

This form is entirely confidential. We do not record any of it. 

1. About Purpose
I haven't given much thought recently to what matters most to me in my life, or about my top priorities.
I haven't really considered aligning my life plan with my sense of purpose.
My retirement focus is more on the career or job that I'm leaving, than on the new life stage that I am entering.  
I have not taken stock of my networks, relationships, skills, capabilities and opportunities which could enable a rich and fulfilling retirement for me.

2. About Community
I should be investing more time in strengthening the relationships I have with the people who care most about me and who I trust.
My network is not big or diverse enough to help me make the best non-retirement decisions.
I lack an experienced and wise mentor to support me in developing my next-stage career.
My conversational skills are not as strong as I'd like them to be, and I struggle a bit to establish and maintain good relationships.

3. About Wellbeing
My self-confidence and belief in myself are a bit low right now.
I sometimes feel anxious and/or depressed about my future.
I feel I should be eating, exercising and sleeping better than I do.
I am generally not that positive and optimistic about my future.

4. About Fulfilment
I feel a bit stuck, and that there's too much holding me back.
I think I know what would give me most fulfilment, but I don't think I'll actually get there.
I have so many responsibilities, and so many people depending on me, that I don't have enough time for myself.
I feel I'm not good enough, or not up to date with things, and that others can do it better than me.

5. About Life Stage
As I get older, I feel unprepared for navigating the next stage of my life.
I'm beginning to feel less valued or relevant at work, and that my most productive years are behind me.
I don't know if I can afford to live and work differently at this point in my life, or whether I will run out of money.
I think my partner/spouse/family have different ideas about my/our future life, such as where we'll live and what we'll do.

Well done! You completed the self-evaluation questionnaire. Some of the questions were challenging, and possibly even a bit provocative or worrying. Do not worry. You are in good company. Add up all your "Agrees". The majority of people "Agree" with more than half of the statements.
